About the Role

Almoosa Health Group is seeking a Population Health Management Officer to join their team in Al-Ahsa. This full-time position is ideal for individuals with 0-1 years of experience looking to contribute to population health initiatives within a healthcare setting.

Core Responsibilities

The Population Health Management Officer will play a key role in supporting and implementing population health management programs. Key duties include:

• Generating and maintaining patient registries, care-gap lists, and high-risk population reports.
• Coordinating patient outreach, recalls, scheduling, and follow-up for screening, chronic disease, and preventive health programs.
• Monitoring program performance, maintaining accurate records, and supporting KPI reporting and dashboard updates.
• Collaborating with clinic staff, Case Management, and Analytics teams to ensure effective program implementation.
• Administering patient surveys, tracking participation, and compiling results for program evaluation.
• Coordinating employer-focused healthcare services, including screenings and occupational health activities.
• Supporting audits, quality initiatives, and value-based care programs.
• Ensuring compliance with information governance requirements, maintaining patient confidentiality, and data security.
• Providing patient-centered support and coordinating care with professionalism and respect.

Qualifications and Experience

Candidates for this role should possess the following qualifications:

• A Diploma in Nursing, Allied Health, Public Health, Health Information Management, or a related healthcare field.
• 0–4 years of experience in healthcare operations, care coordination, case management, quality, population health, or a related area.
• A good command of both Arabic and English languages.

Work Environment and Location

This is a full-time position based in Al-Ahsa. The role involves collaboration across various internal departments to achieve program objectives.
