The Role of Inpatient Case Manager

The Inpatient Case Manager is a pivotal full-time role focused on optimizing patient flow and ensuring effective bed utilization within Al Salama Hospital. This position involves comprehensive coordination of inpatient care processes, from initial admission through to discharge, to enhance both operational efficiency and the overall patient experience.

Key Responsibilities

• Manage inpatient bed operations, ensuring timely and appropriate bed allocation based on patients' clinical needs and hospital capacity.
• Coordinate patient admissions from the Emergency Department, Outpatient Clinics, Operating Rooms, and other referral sources.
• Develop and monitor individualized discharge plans to facilitate safe, timely, and efficient patient discharge.
• Collaborate with physicians, nursing teams, case managers, and allied health professionals to optimize patient flow and continuity of care.
• Coordinate with hospital departments including Admissions, Bed Management, Housekeeping, Billing, Pharmacy, Radiology, Laboratory, and Guest Experience to ensure seamless patient movement.
• Monitor patient length of stay (LOS) and proactively address discharge barriers to improve bed utilization.
• Facilitate multidisciplinary communication to ensure efficient patient placement, transfers, and discharge planning.
• Analyze inpatient occupancy, bed availability, and patient flow metrics to support operational decision-making.
• Ensure compliance with hospital policies, accreditation standards, and patient safety requirements.
• Lead continuous improvement initiatives to enhance patient flow, operational efficiency, and patient experience.
