Clinical Pharmacist

About the Role

Dr. Sulaiman Al Habib (HMG) is seeking a Clinical Pharmacist to join their team. This full-time role involves serving as a pharmaceutical caregiver, focusing on patient-specific pharmacotherapy to ensure optimal outcomes. The Clinical Pharmacist acts as a key resource for pharmacy personnel, medical and nursing staff, and other healthcare professionals, ensuring the effective implementation of pharmaceutical care plans.

Key Responsibilities

• Participate actively in patient care rounds with other team members.
• Assist medical staff in selecting drug therapy and monitoring patients.
• Review and interpret medication orders for clarity, accuracy, rationality, compatibility, feasibility, and suitability for patient needs.
• Assume responsibility for patient-specific pharmacotherapy plans to ensure optimal outcomes.
• Determine workload priorities, verify medication orders, check the final product for accuracy, and ensure timely delivery of medication to patient care areas.
• Act as a drug information and education resource for patients, physicians, pharmacy residents and students, colleagues, medication and nursing staff, and other healthcare professionals.
• Document therapeutic recommendations in the patient’s medical record and pharmacy intervention forms/screens.
• Detect, analyze, and report adverse drug reactions.
• Provide patient and family education as needed.
• Participate in person-centered care initiatives undertaken by HMG.
• Enrich patient experience with compassion, respect, and dignity.
• Perform other applicable tasks and duties assigned within the realm of knowledge, skill, and abilities.

Experience and Qualifications

• A minimum of 4 years of relevant experience is required.
• The Manager of Pharmacy may modify the experience requirement in cases where an individual pharmacist exhibits exceptional knowledge and skills.
