About Johns Hopkins Aramco Healthcare (JHAH)

Johns Hopkins Aramco Healthcare (JHAH) is seeking a dedicated Case Manager 1 to join our team in Dhahran. This full-time role is integral to ensuring seamless patient care transitions and optimizing healthcare resource utilization within our system.

Role Overview

The Case Manager 1 supports senior personnel in facilitating and coordinating the transition of patient care plans between different healthcare environments or to home, based on the patient's need for continued care. This involves assessing the appropriateness and level of care, monitoring the patient's continued stay during hospitalization, and planning for continuity of healthcare upon discharge. The role also coordinates necessary individual, family, hospital, and community resources to implement effective discharge plans, aiming to eliminate barriers to clinical service delivery and monitor clinical resource utilization.

Key Responsibilities

  • Conduct initial screenings for assigned patients to identify strengths and needs related to clinical resource utilization and discharge planning.
  • Collaborate with the healthcare team to develop, follow, and modify multidisciplinary plans of care as required.
  • Identify variances from the established plan of care and facilitate resolutions for the patient, family, and carer.
  • Monitor multidisciplinary patient care activities to develop appropriate care delivery strategies and provide recommendations.
  • Ensure that interventions are aligned with medical management initiatives.
  • Utilize established criteria to determine the appropriateness of admission and the medical necessity of hospital stays for inpatients.
  • Refer cases that do not meet admission or continued stay criteria to the physician reviewer and appropriate administrative staff.
  • Assist outpatients in setting SMART (Specific, Measurable, Achievable, Realistic, and Time-bound) goals for self-management, providing education on self-management tasks and reporting abnormal findings to their physician team.
  • Participate in process improvement activities related to case management practice and integrate research findings into clinical practice.
  • Represent care management in relevant committee meetings, including long stay review, Clinical Guideline Development, Length of stay reduction work groups, and staff meetings.
  • Actively participate in the development and implementation of Case Management projects, such as Clinical Pathways taskforce committees, quality improvement tools, data analysis, and variance reports.

Professional Development and Mentorship

The Case Manager 1 is expected to maintain personal professional development, staying informed of clinical changes and evidence-based practices. This role also involves mentoring and orienting new or less senior staff members on case management roles and responsibilities, contributing to the team's overall growth and expertise.

Qualifications and Experience

  • Current licensure as a professional registered nurse.
  • A minimum of 3 years of clinical nursing experience following licensure/registration in the country of origin.
  • 2 years of Case Manager or Case Coordinator experience is preferred.
