About the Role

Dr. Sulaiman Al Habib (HMG) is seeking a Clinical Pharmacist to join their team. This full-time role involves serving as a pharmaceutical caregiver, focusing on patient-specific pharmacotherapy to ensure optimal outcomes. The Clinical Pharmacist acts as a key resource for pharmacy personnel, medical and nursing staff, and other healthcare professionals, ensuring the effective implementation of pharmaceutical care plans.

Key Responsibilities

Participate actively in patient care rounds with other team members.
Assist medical staff in selecting drug therapy and monitoring patients.
Review and interpret medication orders for clarity, accuracy, rationality, compatibility, feasibility, and suitability for patient needs.
Assume responsibility for patient-specific pharmacotherapy plans to ensure optimal outcomes.
Determine workload priorities, verify medication orders, check the final product for accuracy, and ensure timely delivery of medication to patient care areas.
Act as a drug information and education resource for patients, physicians, pharmacy residents and students, colleagues, medication and nursing staff, and other healthcare professionals.
Document therapeutic recommendations in the patient’s medical record and pharmacy intervention forms/screens.
Detect, analyze, and report adverse drug reactions.
Provide patient and family education as needed.
Participate in person-centered care initiatives undertaken by HMG.
Enrich patient experience with compassion, respect, and dignity.
Perform other applicable tasks and duties assigned within the realm of knowledge, skill, and abilities.

Experience and Qualifications

A minimum of 4 years of relevant experience is required.
The Manager of Pharmacy may modify the experience requirement in cases where an individual pharmacist exhibits exceptional knowledge and skills.
