About the Tamheer - Care Navigator Role

This full-time position involves operational care coordination and patient-facing communication within clinical programs. The Care Navigator plays a crucial role in ensuring timely follow-up, accurate dissemination of medical information, and guiding members through approved healthcare services. The objective is to facilitate coordinated care throughout a member's journey and contribute to improved health outcomes.

Patient Care Navigation and Coordination

The Care Navigator serves as the primary point of contact for members enrolled in clinical programs. Key responsibilities include:

  • Guiding patients through approved care pathways, explaining processes, requirements, and next steps.
  • Coordinating internally to ensure timely processing of healthcare service requests, such as appointments, referrals, and approvals.
  • Maintaining accurate documentation of all member interactions within the system.
  • Collaborating with healthcare providers, patients, and their families to facilitate effective care management.

Clinical Information Communication and Patient Education

This aspect of the role involves clear and accurate communication with members regarding their medical service requests, approvals, and program enrollment status. The Care Navigator will translate medical terminology into easily understandable language while maintaining clinical accuracy. Complex clinical inquiries will be escalated to appropriate clinical supervisors or physicians as needed. Facilitating communication between patients and healthcare providers is essential for ensuring continuity of care.

Operational Support for Clinical Programs

The Care Navigator supports the execution of clinical programs by tracking patient milestones and adherence to assigned pathways. This includes monitoring enrollment, follow-up schedules, and required documentation. The role also assists in coordinating multidisciplinary interactions and arranging necessary resources and services for patients, such as referrals to specialists, community services, and support programs.

Service Request Monitoring and Follow-Up

Responsibilities include tracking healthcare service requests from initiation to completion. This involves following up with relevant departments, including medical approvals, provider network, and customer service, to prevent delays. Members will be notified of updates within agreed service level timelines.

Data Entry, Reporting, and Compliance

Accurate entry of case notes, updates, and program-related activities into internal systems is required. The Care Navigator must maintain confidentiality and comply with healthcare data protection regulations. Basic operational reporting, such as caseload status and follow-up completion rates, will be supported for supervisors.

Patient Experience and Service Quality

The role involves addressing patient concerns related to care navigation within the scope of authority. Recurring operational issues will be identified and escalated to management. Feedback will be contributed to improve communication workflows and patient journey processes.

Required Skills and Experience

Candidates should possess a strong understanding of medical terminology and basic clinical pathways. Familiarity with healthcare authorization and service request processes is necessary. Proficiency in healthcare information systems and documentation tools is required, along with clear verbal and written communication skills.
