About the Role

Almoosa Health Group, located in Al-Ahsa, is seeking a Manager of Population Health Management to lead the strategic design, implementation, and oversight of population health programs. This full-time role involves strategic planning, budget management, and extensive clinical collaboration to drive value-based care initiatives and improve patient outcomes. The ideal candidate will bring 5-10 years of relevant experience to this critical leadership position.

Key Responsibilities

The Manager of Population Health Management will be responsible for developing and executing comprehensive population health strategies. This includes overseeing program development, fostering cross-functional partnerships, and ensuring the effective integration of population health initiatives within clinical workflows to achieve measurable improvements in patient care and operational efficiency.

Strategic Planning and Financial Oversight

  • Develop departmental strategic objectives, Key Performance Indicators (KPIs), and individual employee goals in alignment with leadership direction.
  • Ensure the achievement of departmental targets through effective planning, proactive risk management, and data-driven decision-making processes.
  • Develop, manage, and monitor the annual departmental budget, ensuring alignment with financial plans and minimizing variances.

Program Leadership and Collaboration

  • Lead the design and management of Population Health Management (PHM) programs, encompassing screening, chronic disease management, and employee health initiatives.
  • Define program scope, eligibility criteria, care pathways, identify care gaps, and establish outcome measures in collaboration with clinical specialties.
  • Conduct disease segmentation and risk stratification to identify and prioritize high-risk populations for targeted interventions.
  • Partner with clinical specialties to integrate PHM programs into existing clinical workflows and ensure clinical alignment.
  • Represent PHM initiatives in clinical committees and leadership forums, advocating for population health strategies.
  • Collaborate with Analytics, Case Management, IT, and Quality teams to develop dashboards, KPIs, workflows, and processes for care-gap resolution.
  • Lead PHM contributions to value-based care initiatives, including pilot design and risk-stratification frameworks.
  • Define PHM KPIs, ensuring alignment with measurement logic developed in partnership with analytics teams, and interpret program performance data for executive and board-level reporting.
  • Lead Patient-Reported Outcome Measures (PROMs) initiatives across PHM programs, ensuring integration with quality, patient experience, and IT teams.
  • Support patient-centered care delivery, upholding principles of dignity, compassion, and shared decision-making.
  • Promote continuous improvement through systematic patient feedback collection and outcome tracking.

Qualifications and Experience

  • A Bachelor’s degree in Nursing, Pharmacy, Allied Health, Physiotherapy, Public Health, or a related clinical field is required.
  • A Master’s degree in Public Health, Health Administration, Epidemiology, Population Health, or a related field is preferred.
  • An MD is considered an advantage.
  • Minimum of 5-10 years of progressive experience in population health management or a related healthcare leadership role.
