  • Comprehensive psychosocial assessments and ongoing counseling for residents.
  • Leading interdisciplinary care planning and ensuring goal-directed discharge planning.
  • Advocating for residents’ rights and informed decision-making.
  • Linking residents and families to appropriate community resources and supports.
  • Ensuring ethical practice and regulatory compliance.

Job Requirements:
  • Bachelor’s degree in Social Work or related field from an accredited institution.
  • Valid SCFHS license as Specialist.
  • Additional certifications such as clinical social work or equivalent; specialty certifications in geriatrics, dementia care, palliative/hospice social work; Basic Life Support (BLS) certification.

Key Responsibilities:
