Role Overview

As an Emergency Medicine Consultant at Johns Hopkins Aramco Healthcare (JHAH) in Dhahran, you will serve as a full-time staff member within the Emergency Medical Services Unit. This role requires 2-5 years of relevant experience and involves providing expert emergency medical care for adult, pediatric, and obstetric emergencies. You will be responsible for the efficient operation of your assigned unit, leading resuscitation efforts, and performing critical life-saving procedures as per your privileges.

The consultant provides expert emergency medicine opinions to other JHAH disciplines and remote area clinics, including toxicology presentations. This position also entails responding to disaster or emergency events as a Chief Triage Officer and delivering critical care, including Medevac services. Assignments may extend to any JHAH outpatient or inpatient service across facilities, with a primary mission to deliver emergency medical care to Saudi Aramco and JHAH employees, their dependents, and facilitate the transport of critically ill patients by air or ground.

Core Responsibilities

• Oversee the Emergency Medical Services Unit during shifts, acting as the primary expert.
• Supervise non-EMS specialist clinicians within the Emergency Room.
• Protect JHAH assets by reporting damaged equipment or potential risks.
• Maintain credentials and privileges, pursue continuing medical education, and meet JHAH EMSD requirements.
• Fulfill teaching responsibilities, including participation in medical conferences and various continuing education programs (ACLS, PALS, ATLS, ADLS, NRP, etc.).
• Participate in administrative work as approved by the Chief of EMS.
• Work independently as the primary decision-maker in the clinical setting.
• Advise the company on employee capacity to perform work.
• Act as Chief EMSD or other required roles when assigned.
• Actively participate in Division Quality Improvement (QI) and disaster preparedness activities.
• Provide advice on emergency medicine matters to JHAH Physicians and other healthcare professionals.
• Deliver high-quality emergency medical care, including taking medical histories, examining patients, ordering diagnostic studies, and initiating treatment.
• Interpret data, make diagnoses, recommend/order treatment, medication, and follow-up care, obtaining consultation when necessary, and ensuring complete documentation.
• Administer life-saving and immediate treatment as dictated by patient diagnosis and condition.
• Perform emergency procedures on inpatients as required, such as in response to cardiopulmonary arrest (code blue).

Clinical Procedures and Expertise

The role requires proficiency in a comprehensive range of emergency medical procedures, including but not limited to:

• Airway management: Cricothyrotomy, retrograde intubation, foreign body removal, rapid sequence intubation, mechanical ventilation, percutaneous transtracheal ventilation, capnometry, non-invasive ventilator management.
• Anesthesia and Resuscitation: Local anesthesia (including nerve blocks), procedural sedation, moderate sedation-analgesia, Level 1 resuscitation, blood, blood component, and fluid resuscitation.
• Emergency Diagnostic Procedures: Anoscopy, arthrocentesis, bedside ultrasonography, cystourethrogram, lumbar puncture, nasogastric tube, paracentesis, pericardiocentesis, peritoneal lavage, slit lamp examination, thoracentesis, tonometry, compartment pressure measurement.
• Genital/Urinary: Bladder catheterization (Foley and suprapubic), testicular detorsion.
• Head and Neck: Control of epistaxis, laryngoscopy, drainage of peritonsillar abscess, removal of rust ring, tooth stabilization, lateral canthotomy.
• Hemodynamic Techniques: Arterial catheter insertion, central venous access, intraosseous infusion, peripheral venous cut down, umbilical vein catheterization.
• Obstetrics: Emergency delivery of newborn, emergency cesarean section in arrest situations, excision of thrombosed hemorrhoids.
• Other Procedures: Foreign body removal, gastric lavage, gastrostomy tube replacement, incision/drainage of abscess, violent patient management/restraint, sexual assault examination, trephination nails, wound closure techniques, wound management, escharotomy, thrombolytic therapy, poisoning antidote administration.
• Skeletal Procedures: Fracture/dislocation immobilization techniques, close fracture/dislocation reduction techniques, spine immobilization techniques.
• Lifesaving Thoracic Procedures: Cardiac pacing (percutaneous and transvenous), defibrillation/cardioversion, thoracostomy (chest tube or pigtail catheter), emergency thoracotomy.
• Ultrasound Screening: Triage examination for detecting free fluid in the pelvis, determining fetal viability, and gallbladder stone diagnosis.

Required Equipment Proficiency

Candidates must demonstrate competency in the use of the following equipment:

• Resuscitation, breathing, and circulation equipment.
• Trauma, emergency orthopedic, EYE/ENT, obstetric, urological, and CNS equipment.
• Emergency ultrasound machine for trauma, obstetric emergencies, vascular access, and resuscitation.
• Physical examination equipment, including thermometers, pulse oximetry, portable suction regulator, anoscope, radiological monitoring, and emergency room fluoroscopy machine utilization if needed.
• Disaster communication equipment such as pagers, radios, cell phones, internet, SAP system, SMS, and Emergency Message Board.

Collaboration and Reporting Structure

This role involves frequent contact with patients and their families regarding medical findings and treatment. Close collaboration is maintained with nursing, para-professional staff, and other Consultants. Occasional contact with physicians and hospitals outside JHAH is required for patient transfers. The consultant will also interact with EMS staff, students, and residents for educational programs and supervision. There may be calls to cover VIP events or respond to government needs, requiring high-level interaction with adherence to communication policy.

The Emergency Medicine Consultant reports directly to the EMS Unit Head, who in turn reports to the Chief of EMSD. Daily contact with the Unit Head and ongoing contact with the Chief, EMSD, and JHAH C-suite managers are integral to this position.

Qualifications and Professional Development

Candidates for this full-time position should possess 2-5 years of experience in emergency medicine. New hires will undergo a 3-month assessment period, reviewed by the Unit Head and Chief of EMSD, for presentation to the Credential and Privileges Committee of JHAH to grant practice privileges. Ongoing professional development is essential, requiring the consultant to maintain credentials and privileges, continue medical education, and meet all JHAH EMSD requirements.
