Role Summary

The Senior Pharmacist is responsible for ensuring the safe and effective dispensation and management of medications across assigned JHAH premises. This full-time role involves conducting comprehensive assessments, analyses, and providing recommendations for specific disease management or drug therapy. The Senior Pharmacist collaborates with patient care team members or operates based on approved disease/pharmacotherapy management protocols and JHAH policies and clinical guidelines. Accountability extends to the ongoing evaluation and documentation of all components of pharmaceutical care to optimize patient safety and clinical outcomes.

Strategic Responsibilities

  • Serve as a pharmacy representative for assigned quality improvement teams, unit-based safety projects, or JHAH committees.
  • Participate in service-wide implementation of performance and outcome improvement initiatives to optimize organizational effectiveness, efficiency, and digital transformation, including the development or updates of clinical protocols or conducting medication use evaluations.
  • Conduct and participate in JHAH pharmacy branding activities, such as publishing articles, publishing posters, serving as a speaker in conferences, and participating in community awareness activities to enhance the advancement of the JHAH pharmacy profession.
  • Set priorities, make critical decisions, and respond quickly to emergent continuity of care situations.

Operational Responsibilities

  • Process all applicable medication order-related functionalities at assigned units as per JHAH operational policies, including but not limited to:
    • Order verification
    • Pharmacokinetics dosing
    • Therapeutic monitoring
    • Narcotic stewardship
    • Antimicrobial stewardship
    • Attending clinical rounds
    • Providing patient-centered pharmaceutical care in decentralized/centralized settings
    • Patient counseling
    • Ensuring proper documentation of medication review notes, progress notes, workload, and operational reports
  • Perform pharmacotherapeutic interventions and other patient care-related functionalities mandated by operational need or manager directives on a daily basis.
  • Perform sterile/non-sterile compounding related functionalities at assigned units as per JHAH operational policies.
  • Perform medication stock continuity management functionalities, including but not limited to:
    • Stock replenishment on a regular basis
    • Narcotic stewardship
    • Robotic maintenance
    • Shelf and unit floor stock inspection, maintenance, and replenishment
    • Review usage patterns, shortages, and suggest alternatives
  • Provide specialized didactic and practical education to pharmacy staff, pharmacy interns, pharmacy residents, and other health care professionals.
  • Precept pharmacy interns and residents.

Qualifications and Experience

  • A minimum of 5 to 10 years of relevant experience.
  • Possession of a valid SCFHS license.
