  • Facilitate safe and timely discharge planning, including referrals to home health, SNFs, or community programs.
  • Assess and address social determinants of health affecting care.
  • Maintain accurate documentation in the electronic medical record while ensuring compliance with HIPAA policies.
  • Participate in quality improvement activities and support staff education on psychosocial aspects of care.
