Job Description

About the Role

Baraya Extended Care is seeking a dedicated Social Worker to join its team in Abha, 'Asir. This full-time role involves providing comprehensive psychosocial assessments and ongoing counseling to residents, ensuring their well-being and smooth transitions across care settings. The Social Worker will lead interdisciplinary care planning and facilitate goal-directed discharge planning, advocating for residents' rights and informed decision-making.

Key Responsibilities

  • Provide ongoing counseling and support to residents and their families, including crisis intervention when necessary.
  • Lead or co-facilitate family meetings and care conferences in collaboration with the care team.
  • Conduct advance care planning conversations and document living wills and durable power of attorney for health care decisions.
  • Coordinate multidisciplinary care planning and ensure alignment of social work goals with medical and rehabilitation objectives.
  • Assess and address social determinants of health affecting care, such as housing, finances, transportation, and caregiver burden.
  • Facilitate safe and timely discharge planning, including referrals to home health services, skilled nursing facilities (SNFs), or community programs.
  • Link residents and families to appropriate community resources and financial assistance programs.
  • Educate residents and families about disease processes, treatment options, and available services.
  • Maintain accurate and legible documentation in the electronic medical record, ensuring compliance with HIPAA and confidentiality policies.
  • Monitor and address risk factors, including elder abuse, exploitation, and safety concerns, reporting as per policy.
  • Participate in quality improvement activities and accreditation processes, collecting and reporting data on social work outcomes.
  • Support staff education on psychosocial aspects of care and cultural competence.
  • Maintain professional licensure, participate in supervision, and pursue ongoing professional development.

Qualifications and Experience

  • Experience required: 0-1 years in a relevant social work capacity.
  • Demonstrated ability to conduct psychosocial assessments and provide counseling.
  • Understanding of ethical, culturally sensitive practice and regulatory compliance.

Work Environment

This is a full-time position based in Abha, 'Asir. The role requires collaboration within an interdisciplinary care team to optimize resident well-being.
