About the Role

Health Net Health Plan of Oregon is seeking a Care Coordinator II to join our team in Riyadh, Riyadh, Saudi Arabia. This contract position supports care management activities, ensuring services are delivered effectively by healthcare providers and partners to achieve continuity of care and member satisfaction. The role involves direct interaction with members through outreach activities, both telephonically and via home visits, to document and facilitate their care plans.

Key Responsibilities

  • Conduct outreach to members via phone or home visits to engage them and discuss care plans, including next steps, resources, and ongoing education.
  • Coordinate care activities based on established plans, collaborating with healthcare and community providers, partners, and members/caregivers to accommodate changes or progress.
  • Serve as a primary support contact for member and provider inquiries, requests, or concerns related to care plans.
  • Communicate with care managers, practitioners, and other relevant parties to facilitate member services and ensure continuity of care.
  • Support the performance of service assessments/screenings for members and document their care needs.
  • Assist in documenting and maintaining member records in accordance with state and regulatory requirements, facilitating distribution to providers as needed.
  • Adhere to standards of practice and policies compliant with contractual requirements and regulatory guidelines.
  • Identify member needs and make appropriate referrals to Care Managers, community-based organizations, and Disease Managers.
  • Provide education to members regarding available benefits and resources.
  • Perform other assigned duties and comply with all company policies and standards.

Required Experience and Education

  • Experience: 1-2 years of related experience is required.
  • Education: A High School diploma or GED is required. For the Arkansas Total Care plan, a Bachelor's degree in a social science or health-related field is preferred, or a high school diploma combined with at least one (1) year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients.

Work Environment and Additional Requirements

This is a field-based role that involves supporting members with behavioral health and developmental disabilities needs. The position is designated as safety sensitive and requires a driver's license, child and adult maltreatment check (before hire and recurring), and a drug screen (at time of hire and recurring). Candidates should possess strong experience working with both Behavioral Health and Developmental Disabilities (BH/DD) populations. Travel of approximately 30% within the assigned service area is required.
