About the Care Coordinator II Position
Health Net Health Plan of Oregon, a part of Centene, is seeking a dedicated Care Coordinator II to join their team. This contract position is based in Riyadh, Saudi Arabia, and contributes to Centene's mission of transforming the health of communities, one person at a time.
Role Purpose
The Care Coordinator II supports care management activities and the teams assigned to members. This role ensures that services are delivered effectively by healthcare providers and partners, aiming to achieve continuity of care and member satisfaction. Key responsibilities include direct interaction with members through outreach, documenting care plans, and coordinating service activities.
Key Responsibilities
- Provide outreach to members via phone or home visits to engage them and discuss their care or service plan.
- Communicate next steps, available resources, and address questions or concerns related to recommended care, offering ongoing education as appropriate.
- Coordinate care activities based on the established care plan, working with healthcare and community providers, partners, and members/caregivers to adapt to changes or progress.
- Serve as a point of support for member and provider inquiries, requests, or concerns related to care plans.
- Communicate with care managers, practitioners, and other relevant parties to facilitate member services and ensure continuity of care.
- Support the performance of service assessments or screenings for members and document their care needs.
- Assist in documenting and maintaining member records in accordance with state and regulatory requirements, including distribution to providers as needed.
- Adhere to standards of practice and policies compliant with contractual requirements and regulatory guidelines.
- Identify member needs and make appropriate referrals to Care Managers, community-based organizations, and Disease Managers.
- Provide education on available benefits and resources.
- Perform other duties as assigned and comply with all organizational policies and standards.
Qualifications and Experience
- Education: High School diploma or GED.
- Experience: 1 – 2 years of related experience.
Preferred Skills and Attributes
- Experience in a field-based role supporting members with behavioral health and developmental disabilities needs.
- Strong experience working with both Behavioral Health and Developmental Disabilities (BH/DD) populations.

This is a contract position located in Riyadh, Saudi Arabia. The compensation for this role ranges from $** to $** per hour.
